When does a medusa piercing need to be removed?

My friend got a medusa piercing a few weeks ago and everything healed as it should, but she noticed that the backing of the jewelry has begun cutting the skin that connects lips to gums. Should she take it out?

I recently just got a medusa piercing. It’s pretty angled down, where you can sometimes see the base of the jewelry in the middle of my lips. Can I turn it into a jestrum with slight adjustments?
More so make it more angled. Maybe take the hole already made, and then pierce it to be more angled so the right kind of jewelry can fit in it.
